| 内容記述 | 本稿では、市町村ごとの後期高齢者診療費の格差、地域特性の発生要因の分析と医療費抑制政策について実証的分析を行う。分析からは、一人あたり診療費と高齢化率には負の相関があり、受診率の高さも診療費増加の要因ではない。また市町村ごとの入院・入院外医療費の使い方の違いが、診療費の高低には関係しているものでもない。後期高齢者の平均診療費は、少数の高額医療により引き上げられていることがわかった。市町村別の医療の使い方については、①診療費が高く入院費の割合も高い地域、②診療費が低く入院費の割合が高い地域、③診療費が低く入院費の割合も低い地域、④診療費が高く入院費の割合が低い地域の４種類に分類できる。しかし医療利用の地域差については、疾患等に基づく医学的アプローチだけでは説明がつかない部分も大きく、地域性をもたらす原因究明について医療制度が住民の利用行動に及ぼす影響に着目する新制度論アプローチが有効である。 | |||||

| 内容記述 | This paper studies the regional difference of medical cost and medical consultation behavior of elderly persons who are aged over seventy-five or over in Kumamoto Prefecture. It is used the medical treatment receipts data for this analysis, it is not a Gauss distribution；we perform nonparametric analysis. Medical examination and treatment costs are varying by region than aging degree. There is a negative correlation between medical examination and treatment costs and aging ratio of the municipalities, and the high medical examination ratio is not a factor of the medical examination and treatment costs to increase. In addition, the medical expenses ratio of outpatient and hospitalization has nothing to do with the high and low level of medical examination and treatment costs in municipalities. This means medical examination and treatment costs of the elderly aged seventy-five or over is raised by few patients who need expensive medical services. We may make use of the accumulation curve of the consultation rate for proper policy formation, if we analyze the difference of consultation behavior by age, gender, and municipalities. There are four types of area (1) high ratio of medical examination and treatment costs, and hospitalization (2) low ratio of medical examination and treatment costs, and high ratio of hospitalization (3) low ratio of medical examination and treatment costs, and hospitalization (4) high ratio of medical examination and treatment costs, and low ratio of hospital expense. However, there are many inexplicable social factors only by receipt data analysis. New Institutionalism approach, such as rational choice institution theory, sociological institution theory, and historical institution theory give more persuasive viewpoints to illustrate the citizen's behavior of the medical insurance system usage. |
