@article{oai:kumadai.repo.nii.ac.jp:00029134, author = {生田, まちよ and Ikuta, Machiyo and 生田, まちよ}, issue = {2}, journal = {小児保健研究}, month = {Mar}, note = {application/pdf, 論文(Article), 本研究では、母親の介護負担やその影響の軽減のために在宅人工呼吸療法を行っている小児の自宅に定期的に訪問看護師が長時間訪問してケアを行うレスパイトケアを実施した。ケアを受けた母親の体験を明確にして、このレスパイトケアの意義を明らかにすることを研究目的とした。研究方法は、4名の母親に3~4ヶ月ごとに、半構造化面接を実施して、面接内容を質的帰納的に分析した。その結果、母親の体験は、【母親の介護負担・状況】の中で、【HMV児を委ねる意思を抑制する事柄】と【定期的ホームベースレスパイトケアの効果を導く事柄や変化】が存在し、それらに影響されながら母親は何らかの【気持ちや状況の再構築】を行いながらレスパイトケアを受けて【健康な生き方の回復】を得ることができた。精神的・身体的・社会的介護負担の軽減だけでなく、1人で介護しなくてはいけないという呪縛から解放されやすいことが示唆された。, The purpose of this study was to clarify the significance of long-stay respite care visiting nurses conduct periodically by examining the experience of monthers of children who underwent home mechanical ventilation (HMV). We conducted semi-stuructured interviews every 3-4 months with 4 mothers of children undergoing HMV who received home-based respite care and analyzed interview data qualitatively and inductively. The experiences that mothers had during this period are as follows : they realized the existence of "the matters that inhibit them from leaving their children in the care of others" and "matters that home-based respite care was effective in changing their attitudes on child care" under "care burden and situations for mothers", and then, they engaged in "the restructuring of their own feeling and situations they face" through this experience and finally gained "the recovery of healthy life style" such as enhanced physical and mental state, more involvement in social activities, and the improvement of family relationship.
